Lockout/Tagout (LOTO) violations have surged to the #3 spot in OSHA’s FY2026 preliminary data. The trend is alarming, but the root cause is predictable: organizations are relying on written procedures without enforcing active, floor-level verification.

STC Action Plan

  • Physical Audits: Visually confirm that locks and tags are placed accurately on all required energy isolation devices prior to servicing.
  • Test the State: Ensure personnel attempt to operate the equipment post-lockout to physically verify a zero energy state.

Paper Competence vs. Field Reality in Scaffolding

With Scaffolding citations jumping to #4, organizations are rarely cited for lacking a scaffolding manual. The primary failure point is a stark disconnect between the “competent person” listed on paper and the actual oversight occurring on the job site.

  • Unauthorized modifications by sub-contractors (e.g., removing cross-bracing or guardrails).

  • Employers continue to neglect providing essential safety measures like guardrails, safety nets, or personal fall arrest systems, leaving workers at risk of severe injuries or even death.

OSHA cited a New Jersey vegetable processing facility for willfully and repeatedly violating safety requirements after a worker was fatally injured while cleaning and sanitizing a machine. The onsite temporary employment agency was cited for three serious violations with proposed penalties of $33,100 for not implementing or training workers on lockout/tagout procedures.

Falls are widely known as the leading cause of death in the construction industry. A fact sadly illustrated by the death of a 21-year-old laborer, who fell nearly 50 feet as he installed a supported tubular welded frame scaffold. An OSHA investigation determined the employer failed to ensure the laborer’s fall arrest harness was attached, as required.
